WHAT TO DO WHEN AN EMPLOYEE IS INJURED:

STEP 1. TAKE CARE OF THE EMPLOYEE’S NEEDS FIRST– • Administer First Aid as needed.

STEP 2. IF THE EMPLOYEE REQUIRES EMERGENCY TREATMENT- •

STEP 3. IF NON-EMERGENCY MEDICAL TREATMENT IS NEEDED OR DESIRED– •

If the employee’s injury requires medical attention or you wish to have the employee’s injury checked by a physician, refer the injured employee to either of two Occupational Medical Health Clinics:

Or to his or her personal physician if they have a Pre-Designation of Personal Physician* form on file

• Call Concentra - Gilroy (408) 848-0444 or Concentra – San Jose (408) 477-8080. Let the receptionist know that you are calling from the City of Morgan Hill and that you are sending your injured employee for treatment. Let the receptionist know how to contact you if needed. • Contact Human Resources after initial contact with clinic so Human Resources can authorize the visit. Please contact Michael Horta or Elysa Olivares for after hour authorization. • For critical care and/or after business hours send the employee to St. Louise Regional Hospital Emergency Room, 9400 No Name Uno, Gilroy, CA 95020, (408) 848-2000.

WHAT TO DO AFTER THE EMPLOYEE HAS BEEN CARED FOR:

STEP 4. IF THE EMPLOYEE REQUIRES ONLY FIRST AID FROM AN EMPLOYEE OR A PHYSICIAN - • First Aid is defined by the type of treatment rendered, not who renders it. Employees can render First Aid at work or a physician can render First Aid when an employee is seen at his/her office. If First Aid is the only treatment rendered and a physician clears an employee for full duty, Supervisors should complete the City of Morgan Hill Incident Report and return it with the entire packet to Human Resources as soon a possible. • If an employee is seen by a physician, follow-up with Concentra and tell them that you would like a work status report upon completion of the employee’s treatment. Request that the office call you as soon as possible with the employee’s work status, and that the clinic/doctor should provide the employee with a specific and clearly written work status/restrictions/instructions report.
