E MPLOYEE P ERFORMANCE E VALUATION F ORM I NSTRUCTIONS

Employee evaluations are an important tool used by supervisors and managers to periodically document an employee’s performance. They are necessary to justify salary increases or other personnel actions and are a resource upon which to base career advancement judgments. They are an equally valuable source of feedback, encouragement, and direction to the individual employee. Regardless of what some employees may say, documenting good performance recognizes hard work and pointing out development or improvement needs serve as a guide to solid performance or career advancement and is appreciated by the employee. Performance Evaluations should be the documented result of a performance related conversation between the supervisor and employee. Performance Evaluations need not be long or time consuming to compose. They do need to be complete and based directly on work related performance dimensions or expectations that are valued by the City. This performance evaluation form attempts to make the supervisor’s job of evaluating employees brief and straight-forward in a two page format, while preserving the need to document all needed information Employee Information: Complete the top portion of the Evaluation Form which includes employee information, the rating period, and the reason for the Evaluation. Evaluations are due within the last week of the evaluation period. They must be submitted to the Human Resources Department promptly because the step increase will be processed at the time it is due. If the performance evaluation is not going to be done in a timely fashion, Human Resources must be notified if an employee’s performance does not meet the qualifications for a step increase. Performance Dimensions: When preparing a performance evaluation, supervisors should collect and review work observations, notes, commendations, improvement needs, employee goals and other information from the previous rating period. Check the appropriate rating box pertaining to each Performance Dimension. Each Performance Dimension includes key words or concepts that should be applied to the individual employee’s job duties or assignment. Check the appropriate rating box for Overall Performance. This overall rating should not necessarily be the “average” of the other rating boxes. Rather it is the supervisor’s assessment of the employee’s total performance considering all factors combined. Signature Block: Have all parties sign the evaluation after it is reviewed. Check the appropriate recommendation box if a salary increase or other personnel action should result from evaluation. Should the employee refuse to sign the evaluation, the supervisor should note “refused” and initial the employee signature line. P AGE 1:
