Training Received during Rating Period: List any formal training received by the employee.

Projects Assigned during Rating Period: List any special or key projects that the employee was involved in during the rating period. Depending on the employee’s regular job duties, these projects may be special assignments or a listing of major, regular work assignments that were undertaken during the rating period. Narrative Comments: Narrative comments are the descriptive core of the evaluation process. However, they need not contain lengthy, generalized statements of an employee’s performance. Rather, these comments should be brief and summarize the employee’s performance based directly on the supervisor’s notes and observations. Comments should include specific examples which support the ratings given on Page 1. This section should also contain productivity statistics (if applicable), references to achievements, progress toward employee goals established at the beginning of the rating period, as well as references to compliments, commendations, counseling or disciplinary action the employee received during the rating period. Note that written commendations, notices of disciplinary action or other documents should not be attached to the evaluation itself, but should be maintained separately in the employee’s personnel file as required by law.

While most narratives can be brief, some may take extra space and should be attached on a separate sheet.

Development Goals and Additional Needs: This section should list employee goals, training objectives or other development steps to be taken during the next rating period.

Employee Comments: Employees have a right to comment on their performance evaluation. An employee may enter comments in this space, or may provide a separate written document that should be attached to the evaluation.
