• Symphony pump: available for critically ill infants or VLBW infants in the NICU • Full time/part time student or for employment (no minimum hours)

• Medication use-temporary cessation of breastfeeding

• Other reasons with approval from the BF Coordinator or from the IBCLC • Special circumstances Working or Student Mothers: Medela Pump in Style and Hygeia Enjoye • Working or student mothers (minimum of 20 hours) may qualify for a personal pump • Personal pump issued when hospital grade pump is swapped after 1 month • Participant may not receive formula for 6 months after receiving a personal pump

Medical Grade Electric Breast Pump Loans: Medela–Lactina, Symphony or Ameda • Separation from the infant (i.e. hospitalization of mom and/or baby) • Baby unable to sustain latch and feed well at the breast

• Special circumstances: i.e. cranial facial abnormalities, multiples, etc. • Mothers who are sick and/or using short-term contraindicated medications • Full time/Part time employment (minimum of 24 hours per week) and students (enrolled in 8-12 units) who are exclusively breastfeeding • Hospitals may fax a referral form directly to the PHFE WIC BF Helpline at (626) 208-1338. Working or Student Mothers–Medela Personal Double Pump • Issued when the support pump is swapped for the personal pump at about 6 months; no new application needed.

Hospital grade Medela, Ameda and Hygeia; Medela Harmony manual pumps • Those eligible must be a Watts WIC participant and meet the following criteria: – Hospitalized, premature or critically ill baby who is unable to nurse

– Hospitalized mom or infant

– Infant with severe feeding or suction problems, i.e. cleft lip/palate – Other reasons with approval from the IBCLC or BF Coordinator – Discharged and transitioning to the breast • Participants can come into the office and request an electric pump. They do not need a referral or documentation from the employer or doctor as long as they meet the criteria. Participants can receive a pump at the time of their visit if in stock. – Mom returning to work or school and BF 100%
