For each combination of treatments:

• Divide mortality rates for those receiving all the treatments by the rate ratios from the trials for each individual treatment in turn. • Hence estimate mortality rates in the absence of each treatment in patients judged suitable for it • Then estimate cumulative risk for patients without that treatment.
