6.09 Trauma Skills I Station: Bleeding Control / Shock Station Instructor Guidance

Objectives: 1. Demonstrate emergency medical care of the patient with external bleeding using direct pressure. (pp 788–789, Skill Drill 23-1) 2. Demonstrate emergency medical care of the patient with external bleeding using a commercial tourniquet. (pp 790–792, Skill Drill 23-2) 3. Demonstrate emergency medical care of the patient who shows signs and symptoms of internal bleeding. (pp 794–796, Skill Drill 23-4) 4. Demonstrate how to apply a pneumatic anti-shock garment (PASG). (p 1031–1032, Skill Drill 29-9) Setup: You will need your cohort bag. You will also need special dressings: - 4x4 gauzes - 5x8 gauzes - Multi-Trauma Dressings - Tourniquet - Blanket (space or regular blanket) Deployment: DO NOT LECTURE – your students should be working hands-on within 5 minutes of arriving Deploy your students in teams of 2 students. One will be the patient, the other the rescuer. Have them treat the following isolated injuries (they do not need to do patient assessment): - Laceration to hand from cutting a bagel (ends with direct pressure/elevation) - Head laceration to right forehead from fall (ends with direct pressure/elevation) - Small burn 2”x2” to forearm from stovetop (pain is relieved with commercial WATERGEL dressing - Serious bleed from severed hand from getting caught in woodchipper (bleeding does not stop with direct pressure/elevation but does stop with tourniquet) - Serious bleed from gunshot wound to thigh (bleeding does not stop with direct pressure/elevation but does stop with tourniquet) - Internal bleed for a patient who was stabbed in the stomach. There is no external bleeding but the abdomen is distended and rigid/painful. MAST/PASG needs to be applied: o Patient is log-rolled onto PASG/MAST o Velcro is closed around legs and abdomen o Leg compartments inflated until Velcro crackles o Close leg compartments o Reassess blood pressure and heart rate o If signs of shock are still present inflate abdominal section of garment o Close abdominal compartments o Reassess blood pressure and heart rate o Remove foot pump
