6.15 Instructor Guidance – Evaluating the Vest Extrication Device

Objectives:

1. Demonstrate how to apply a cervical collar to a patient with a suspected spinal injury. (pp 917–918, Skill Drill 26-5) 2.. Demonstrate how to immobilize a patient with a suspected spinal injury who was found in a sitting position. (pp 911– 914, Skill Drill 26-3) 3. Demonstrate the assessment of neurovascular status. (pp 1015–1017, Skill Drill 29-1) 4. Demonstrate the proper use of a Vest Extrication Device. - Chair with no arms - Vest Extrication Device with 2 head straps - Collar - One student should serve as patient. Another student as head immobilizer. Only a third student performing the procedure should be evaluated. Instructions to be read to students: “Your patient is a 43 year old male involved in a head-on motor vehicle collision. You have completed an assessment and find that he is complaining of back and neck pain, but is stable. Please perform the isolated skill of vest immobilization as per the skills sheet.” Setup:

Precautions: Do not lead the student or correct performance. This is a testing station and no feedback should be offered.

Proofing Points:

Two straps – forehead and chin

Head assembly covering at least 50% of ear and not touching top of shoulder (unless patient is super-tall)

Chest assembly is midline and tight

Device is noticeably high in the armpits

Groin straps are not crossed

Please note the following:

- Asking males “what side they are dressed on?” is not a thing. Don’t demand it in station.

- The only required strap order is TORSO > LEGS > HEAD as identified in the manufacturer’s guidelines. Please do not test for more complex “My Baby Looks Hot Tonight”-type sayings.

- Insure neurovascular status is assessed before and after the splint
