DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR HAWAII

This Policy Supplement applies only to those Associates working in the state of Hawaii, excluding DOT- Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with the Hawaii drug and alcohol testing law and regulations. Right to Refuse: Each person must be advised at the time of testing that they have a right to refuse. The Company may take employment action for refusing. Advance Written Notice: Each person tested must be given advance written notice specifying the drugs to be tested and that over-the-counter and/or prescribed medicines may cause a positive test result. Collection Sites: The Company will only contract with and allow collection of samples by qualified individuals and in a laboratory qualified/licensed per Hawaii law and regulations. Drug Screening Cut-Off Levels that differ from the Company Drug Screening Standard : Type of Drug Initial Test Cut-Off Level Confirmatory Test Cut-Off Level Amphetamines (meth) 1,000 ng/mL 500 ng/mL Cocaine 300 ng/mL 150 ng/mL The Company may add substances permitted by law. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request.

Medical Marijuana Be advised that even though the medical use of marijuana has been authorized in this state, employers have not been required to accommodate its use in the workplace. The Company will abide by all related state laws.

All Hawaii Associates must be given a copy of the Company’s Drug & Alcohol Testing policy along with this State Supplement and sign the Drug & Alcohol Testing Consent Form.
