DRUG & ALCOHOL TESTING POLICY – STATE SUPPLEMENT FOR IOWA

This Policy Supplement applies only to those Associates working in the state of Iowa, excluding DOT-Drivers . Where state law requires greater or separate requirements than outlined in the Company’s Drug & Alcohol Testing Policy, this Supplement will be followed in accordance with Iowa drug and alcohol testing law and regulations. Posting Requirement: Confidential access to information for any available Employee Assistance Program services will be posted in a visible location where all other workplace posters are displayed at the worksite location. Advance Written Notice: A copy of the Company policy and this Supplement will be provided to each Associate. The policy will be available for Associates and Conditional Offers to review and copy. Parents of any person under the age of 18 will also be provided a copy. Conditional Offers will also receive written notice of the policy upon hire and before testing. Acknowledgements will be required for each Associate acknowledging that the policy was provided to him/her. Collection Sites: All drug and alcohol test samples will be collected according to state required procedures and analyzed by approved laboratories using authorized approved test methods and technology. Drug test collection will all be made a split sample collection. Post-Incident Testing: Post-accident testing may only occur if it results in a chapter 88 recordable injury or more than $1,000 damage to property. Soft-tissue injuries (e.g. carpal tunnel) are not “accidents” under this policy. Post-Test Procedures: For any positive test result the Company will send by certified mail, return receipt requested , written notice of the result to the Associate and information regarding the Associate’s rights to have the “split” portion of the sample tested at a lab of the Associate’s choosing. Any Associate who is under the age of 18 and the Associate’s parent/guardian will be notified by certified mail, return receipt requested of any positive test result and the Associate’s rights. Consequences: The disciplinary provisions of the Company policy will be followed. However, anyone with an alcohol result of 0.04 or above who has worked for the Company 12 of the past 18 months will be offered an opportunity at counseling. If no other benefit program is available, the Company will assist with the cost of treatment. Access to Records: All test related records (including test result) will be made available for copying to Conditional Offer and Associate upon request. Required Training: Same as Company policy but supervisors will at a minimum receive an initial 2 hour and annual 1 hour training concerning the recognition of employee alcohol abuse and drug use, how to document it, how to corroborate it and how to appropriately make referrals for assistance.

All Iowa Associates must be given a copy of the Company’s Drug & Alcohol Testing policy along with this State Supplement and sign the Drug & Alcohol Testing Consent Form.
