Rights and Responsibilities of the Goodman, Ozark and Seneca Telephone Companies and our customers

The Goodman, Ozark and Seneca Telephone Companies will issue a bill not less than every 28 days or more than 31 days (except for number changes), outlining your toll calls, local service charges and federal state and local taxes. Bill is due the first of each month, and will become delinquent on the 21st. In the event your bill becomes delinquent, a five-day notice will be sent before suspending your service. A restoral of service charge will be applied if your service is suspended. Under our billing procedures and subject to rules established by the Missouri Public Service Commission, we may require a deposit or a guarantee of payment. In the event your bill becomes delinquent when a member of your family has a medical emer- gency requiring the continuance of your telephone service, please call the business office and we may continue your telephone service during such an emergency. The telephone number for GOODMAN is 364-7214 and for LANAGAN customers is 364-7214 . The telephone number for SENECA & TIFF CITY customers is 776-2247 . The telephone number for NOEL is 475-2211 and Southwest City is 762-2211 . Arrangements can also be made with our business office to avoid discontinuance during a period of absence from your home. We will make every effort to clarify and resolve all disputes. Our business office is open from 8:00 a.m. to 5:00 p.m. Monday thru Friday, for handling inquiries by telephone or by personal visit. These telephone companies are regulated by the Missouri Public Service Commission, and you can make an informal complaint in writing or by telephone or personal appearance to the Commission without the need of legal assistance. The Commission’s office is located at 200 Madison Street, Jefferson City, Missouri 65102, and their toll-free number is 800-392-4211. The Office of Public Counsel’s telephone number is 573-751-4857 or toll free 866-922-2959.

Arkansas Customers of Ozark Telephone Company

Payment of bills may be made in person at our busi- ness office, by mail or by previously arranged bank draft. Delayed payment agreements are available to customers who are unable to pay a bill in full. Agreements can be made to delay termination of a delinquent bill due to serious illness and to the eld- erly and handicapped. Lifeline/Link-Up Service is available to qualifying low income residential cus- tomers meeting the state guidelines. These arrangements can be made with our business office. Vacation rate is available for extended absence from home. Your number is reserved for $5.00 per month. Bills can be mailed to an alternate address or payments made in advance. If you are unable to solve problems by calling our business office, you may contact the Arkansas Public Service Commission, 100 Center St. P.O. Box 400, Little Rock, AR 72203. The toll free number is 1-800- 482-1164 or 501-682-1718. The Public Service Commission rules and tariffed rates for service are on file with the Arkansas Public Service Commission and are available to the customer on request at our business office in Seneca, Missouri. Arkansas Public Service Commission Requires Public Utilities to Provide the Following Information.
