A coordinated and integrated program where multiple professionals share responsibility in evaluating, planning and implementing supports to improve capacity of a child with disability or developmental delay and their family. Provision of interventions by more than one professional in a group session towards the participants agreed goals. Group of three. Program to empower participants and improve interactions between participants and their social networks. Assistance to engage effectively in the community through a group approach to help achieve goals, gain insight into their lives and make informed decisions. Assessment, therapy, training, fitting and any approved travel to deliver support. Multidisciplinary model of support delivered to participants over six years to improve community outcomes for participant.

OUTCOME DOMAIN: COORDINATION OF SUPPORTS Specialist Support Coordination 07_004_ 0132 _8_3 Other

The provision of support coordination within a specialist framework necessitated by specific high level risks in the participant’s situation. Support is time limited and focuses on addressing barriers and reducing complexity in the support environment, while assisting the participant to connect with supports and build capacity and resilience. It may also involve development of an intervention plan which will be put in place by disability support workers. When profession = other supports are delivered by allied health professionals with extensive experience in support coordination for people with complex needs.
