Enclosed you will find important information regarding your employee benefits as well as company policies & procedures. Please read through this information carefully. All required forms can be found here. Please complete and submit as soon as possible.
Risk Management Forms – Complete and submit immediately
Background Check Release Form Consent to Drug Testing Statement of Understanding Payroll Forms – Complete & submit immediately
I-9 Form W-4 Form Direct Deposit Form 401(k) EZ Enrollment form (optional) 401(k) and ESOP Beneficiary assignment (optional) EEO Self-ID form (optional) Employee Handbook Form – Complete & submit immediately
Acknowledgement of Receipt – Turn in paper form to Payroll or HR
Employee Benefit Enrollment – This process is now completed online with minimal paperwork! New Hire Information sheet – submit immediately to firstname.lastname@example.org for account creation. You will receive notification via email once your account has been created. Call the Benefits Service Center at 844-315-3799 to enroll in your benefits. You have 31 days from your hire date to complete the enrollment. If it is not completed within 31 days, you waive all rights to benefits.
