Plan Overview:

The following is a general description of the proposed benefits. The complete terms and conditions are shown in the policy.

Yearly Renewable Term:

• Yearly Renewable Term Life Insurance Policy, Generic Form Number: 17-YRT. • Level death benefit to age 95. • Annually increasing, non-guaranteed premium to age 94.

• This policy is convertible without additional underwriting to any single life, annual premium whole life plan, or any other permanent insurance plan that Guardian makes available for this purpose. See the Extended Conversion Rider information below for details. • Disability Waiver Of Premium Rider, Generic Form Number: 17-WP YRT. • If the insured is disabled prior to age 60, premiums will be waived until the disability ends. If the insured is still disabled at age 65, premiums will be permanently waived. • If the insured is disabled on or after age 60, while disability continues premiums will be waived for the later of 2 years or until age 65. • If the insured is disabled prior to age 60, an "own occupation" definition of disability applies for the earlier of the first 7 years or age 64. If the disability begins at or after age 60, the "own occupation" definition applies for the later of 2 years or age 65. • The premium is payable to age 65. The rider terminates at age 65 unless premiums are being waived.

Waiver of Premium:

Extended Conversion Rider:

• Extended Conversion Rider, Generic Form Number: 17-ECR YRT. • This rider extends the conversion period to age 65. • If this rider is not elected, the conversion period would be 5 years.

Accelerated Terminal Illness Rider:

• Accelerated Terminal Illness Rider, Generic Form Number: 17-TABR YRT. • The policyholder may elect to receive a portion of the death proceeds if the insured is terminally ill as defined in the rider. This rider has no premium.
