How to identify the palliative care patient ?

Indications to start palliative care : 1-Surprise question: “Would You be surprised if this patient is dying within 6-12m?” or 2-What are the wishes and needs of this patient ? or 3- Are there clinical indicators of progressing disease: cancer – organ failure - frailty ( ±dementia)
