20. HEALTH INSURANCE AND OTHER BENEFITS

Merritt offers a group medical program to employees as deemed appropriate. If eligible, benefits are effective the first of the month following sixty (60) days of employment. In addition to group health insurance, at this time, Merritt also offers eligible employees, beginning the first of the month following 60 days of employment, dental and vision insurance, life and accidental death and disability insurance (1st of month following 6 months of employment), 401(K) plan with an employer match and a Flexible Spending Account. For details of the health insurance and other plans, please refer to the information given to you at the start of your employment or sent as updates. These benefits may change over time depending on business and other conditions. 21. PAID TIME OFF (PTO) BENEFITS Customary PTO schedule is as follows, but may be adjusted at the Team Leader’s discretion:

Upon Hire - 16 hours After 90 days of employment - 40 hours (total) 6 months to 1 year - 104 hours (total) 1 to 4 years - 144 hours (total) 5 to 9 years - 184 hours (total) 10+ years - 224 hours (total)

Your paid time off (PTO) accrues on the 1st of every January. When the date of hire is in the months of July – December, you receive the PTO allotment the following year. PTO must be used in the year in which it was granted and cannot be carried over to the next year. Scheduling PTO: Use of PTO requires advance approval by your Team Leader. Keep in mind that certain times of the year are very popular for vacation requests and that you should request PTO during those periods well in advance. If you resign during the year and have not used all of your available PTO, you may be eligible to receive pay instead of leave, provided that you give proper notice of resignation, work as scheduled until your resignation date, pay off any debts to Merritt and return all Merritt property (for example, keys, manuals, equipment). Employees who are terminated by Merritt or who fail to give proper notice of a resignation may not be eligible for pay for unused PTO.
