Based on your 2019 KYN: • If BP is > 135/85, lower BP to < 135/85. • If BMI is > 30, lose 5% of your weight or reduce BMI below 30. • If tobacco user, attend one smoking cessation class. *Lifestyle Consult must be complete to earn improvement credit.

Certify your blood pressure, BMI and tobacco status by asking your provider to complete the Wellness Activity Certi cation Form and submitting to Health Engagement.
