STEPS TO OBTAIN A CERTIFIED COPY OF AN ORIGINAL DOCUMENT

STEP ONE (1) The applicant makes a photocopy of the original document. This is the copy that is to be certified as “a true copy of the original document” by the applicant’s supervisor/manager or proxy. STEP TWO (2) The applicant provides their supervisor/manager or proxy with the original document and a photocopy of the original document. STEP THREE (3) Each page of the document is checked against the copy by the supervisor/manager or proxy to ensure that it is a true copy of the original document. STEP FOUR (4) The supervisor/manager or proxy certifies the photocopied document by adding their signature, full name, company name (as applicable), position/title and date to each page. STATUTORY DECLARATIONS A CMSA UAE Statutory Declaration Form may be used in lieu of the Employment Verification Form should an applicant be self- employed and/or need to reference multiple employers/appointments for the FTE (hrs) of vocational case management experience pursuant to the nominated pathway. 1. the total hours of your vocational case management experience - this must be equal to or greater than the hours as stated within your nominated Pathway. For example: Pathway One (1): two (2) years FTE (3648 hrs); Pathway Two (2): three (3) years FTE (5472 hrs); Pathway Three (3): five (5) years FTE (9120 hrs); and Pathway Four (4): two (2) years FTE (3648 hrs) 2. your vocational case management experience in chronological order commencing with your current/most recent position including: the dates you commenced and ceased employment in each position (in month and year format MM /YY), your position title, your employers name (company name) and location (area, emirate state/province, country). AUDITING OF APPLICATIONS As part of the CMSA’s ongoing quality assurance of Certified Case Managers™ the Society will conduct random audits of applications, including authenticating information provided by the applicant or on the applicant’s behalf within the Employment Verification Form, Professional Training and Development (PTD) Form A and Form B, Referee Form 1 and Referee Form 2. Should you be audited you, and/or individuals submitting information in support of your application, will be given 28 days, from the date of notice issued by the CMSA, to verify, clarify and/or substantiate all claims. This may include a request for evidence (e.g. certified copies of certificates, letters of verification etc) of claims you have made as to your Professional Training and Development activities.
