¹A CMSA UAE Statutory Declaration form may be used in lieu of the Employment Verification Form should an applicant be self- employed and/or need to reference multiple employers/appointments for the FTE (hrs) of vocational case management experience pursuant to the nominated pathway. Please refer to STATUTORY DECLARATIONS on page 18 for further information.

PART B

A certified copy of the following qualifications (as applicable) may be required depending on the vocational pathway you have nominated in your application for certification. Please refer to the list below to identify any qualifications that you are required to submit to the CMSA upon application. You are required to make a photocopy of the original qualification and have this copy certified. This copy should then be scanned and saved to your personal computer in readiness to attach to your online application. • Pathway 1 - PhD or Masters Degree or CHC82015 Vocational Graduate Certificate or KPMG Case Management Fundamentals • Pathway 2 - Bachelor Degree or Graduate Certificate • Pathway 3 - Diploma or Certificate (this is not applicable where an applicant holds no formal qualification) Where a copy of a document (i.e. form or qualification) is required to be certified as a true copy of an original document, it must be certified by one of the following: • A direct line supervisor or manager of the applicant • A proxy – this option is available to applicants who do not have a direct line supervisor or manager. For example, a Director of a Company. When nominating a proxy the applicant must select an individual of seniority, integrity and of good standing and reputation in United Arab Emirates. We recommend the applicant contacts the CMSA to obtain their feedback and approval prior to proceeding with engaging the services of the nominated proxy. GETTING DOCUMENTS CERTIFIED
