Case study for individual management plan

Case study for individual management plan

A new child, Tommy Jones, is enrolling to attend the Greenwood pre-school centre. Tommy is a 4 year old by who has been diagnosed with asthma and anaphylaxis.

His asthma is triggered by pollen, however it is well managed and not usually severe if he takes his reliever medication when symptoms appear. His anaphylaxis is due to a severely sensitive bee sting allergy. Tommy’s parents, Peter and Anna Jones, have participated in an enrolment interview with Jenny Boss, the principle of the centre, and have provided the centre with Tommy’s personal actions plans with clear instructions for first aid and emergency management. His doctor has created separate plans, one for asthma, and a separate ASCIA action plan for anaphylaxis. They have also provided medications, his EpiPen adrenaline autoinjector and his blue/grey asthma Ventolin reliever puffer, mask and plastic spacer. His plan will need to articulate prevention strategies to avoid exposure to his known triggers and ensure his safety. It will also need to include emergency contact details and what to do in case of an emergency. You will use medical information provided by his parents, the action plans prescribed by his doctor and the schools policies and procedures to develop his plan. You will need to consider in your plan that certain areas of the pre-school grounds have flowering plants which attract bees in spring. The groundskeeper conducts ground maintenance, grass mowing and gardening throughout the day. The school policy is to locate a copy of the individual management plan in the person at risks (Tommy’s) classroom, clearly displayed on the wall near the door, and next to a secure medicine cupboard (out of reach of children) which will house his medications. A copy will also be required to be kept in the main office and also the first aid room with the general use anaphylaxis and asthma first aid kit/s, so these resources can be easily located no matter where Tommy is in the pre-school grounds. You will be his teacher, and as such have been tasked with creating an individual management plan for Tommy.
