2A Support Clients to make informed decisions through the provision of information and available options regarding an identified need.

Documentation by CM demonstrates the provision of information (both internal and external to the program) by the CM to the Client. CM can demonstrate knowledge of available services which are appropriate to Client goals. CM researches local and regional networks and new services to enhance the CM’s knowledge of services and supports to achieve Client outcomes. CM documentation shows ongoing discussion with Client regarding service options including eligibility requirements for any supports.

CM does not source or provide information to the Client.

CM is unable to demonstrate either

knowledge of services or a strategy to determine appropriateness of services. CM cannot demonstrate research into local networks and new services.

CM researches national and international networks and new services to enhance the Case Manager’s knowledge of services and supports to achieve Client outcomes.

CM researches statewide networks and new services to enhance the CM’s knowledge of services and supports to achieve Client outcomes.

No documentation. No evidence Client is involved in the decision making.
