2. The Address and Phone Change screen displays. Enter the cardholder’s name. One you enter the cardholder’s name, the Submit button will appear.

3. Select the request type from the drop-down menu and add a note to the Memo field. The request type drop-down options are: ➢ Address Change – When selected, the primary and statement address fields are displayed. ➢ Phone Change – When selected, the phone number fields are displayed. ➢ Address and Phone Change – When selected, the phone number, primary address and statement address fields are displayed. a. Phone Change Request – Change or add the phone number in the appropriate field and click Submit .

b. Address Change Request – Update or add the address in the appropriate Primary Address and/or Statement Address fields and click Submit .
