ENGINEERED SOLUTIONS OF GEORGIA PROPOSES TO FURNISH AND INSTALL THE FOLLOWING SCOPE OF WORK:

BEAM REPAIR PLAN: = $6,825.00 INCLUDED IN CONTRACT COST

1. Install temporary supports under floor joists on each side of existing beam. 2. Remove existing deteriorated beam and 2x4 ledgers and haul away. 3. Install new (2) 2x10 beam and 2x4 ledger secured with Trusslok screws. 4. Install (1) new single code compliant 3-inch, schedule 40 steel support column with screw adjuster, 6”x6”x3/8” top cap and 6”x6”x1/2” bottom plate under mid span of new beam and secure top plate beam and anchor base plate to footing. 5. Remove temporary supports and remove all construction related debris. INSTALL (9) GALVANIZED FOUNDATION PIERS WITH FOOTINGS: =$15,875.00 INCLUDED IN CONTRACT COST 1. Have utilities marked by locating service. Locating service marks from street to meter. If any private utilities are suspected in the work area, a private locating service must be contracted by owner. ESOG may provide recommendation upon request. 2. Excavate area to expose the underside of granite foundation and pour new steel reinforced 3000 psi concrete footings. 3. Prep footings and attach galvanized foundation brackets for each pier under center load of footing. 4. Drive foundation piers up to 21’ through poor soil to load bearing strata (see terms and conditions on page 2 for additional charges assessed if piers advance past 21’). 5. Transfer load to piers to lift and stabilize foundation against any further settlement. 6. Secure piers, backfill holes, clean work area and remove all construction related debris. OPTION TO ADD 3 ADDITIONAL SUPPORT COLUMNS AND MID SPAN BEAM AND SISTER 3 JOISTS: =$2,790.60 INCLUDED IN CONTRACT COST

NOTES: ESOG cannot guarantee foundation wall will lift back to its original position and cannot be responsible for any cosmetic damage due to lifting. Homeowner is responsible for any cosmetic repairs. Scope of work has a transferable life of the structure warranty. 25% is due upon confirmation. See terms and conditions for additional notes.
