401 ACA Mandatory Marketplace Disclosure (Part A and B)

An employee whose regularly scheduled work week is less than 20 hours per week is not eligible for fringe benefits, such as paid time off or health, disability, and life insurance coverage. Interns and students are not eligible for any fringe benefits. 403 Company-Issued Credit Cards ITU AbsorbTech will, at management’s discretion, issue a Company credit card or travel advances to managers who will regularly incur customer entertainment or business travel expenses. Travel advances will be provided to those employees without a Company credit card. Abuse of credit card privileges is not tolerated and disciplinary actions are taken as deemed appropriate. Lost or stolen cards must be reported to the accounting office immediately. Managers are required to review expense reports thoroughly to verify that requested reimbursements make business sense and adhere to this policy. Any unusual reimbursement request should be fully explained by a General Manager, in writing, and attached to the weekly expense report when forwarding to the accounting office. 402 Benefits Eligibility Policy

ITU AbsorbTech offers a benefit plan to full-time employees on the first of the month following 60 days of full-time employment. Any employee that regularly works 30 hours or more each week is considered a full-time employee. Each full-time employee will receive the specific information related to their benefits eligibility date and the specific benefits options in which they are eligible to enroll from Human Resources. Any employee whose regularly scheduled work week is more than 20 hours per week, but less than 30 hours per week, is considered a part-time employee and will only receive benefits on the following basis: • Holidays, vacation days, personal days, jury duty and funeral leave are paid pro- rata based on your regularly scheduled working hours and rate of pay. An employee who works 20 hours per week will have a regular daily wage of 4 hours times their hourly rate.

SECTION 4

• Sick leave pay for hourly employees is paid at 7/8ths of the employee’s

regular daily pay rate; pro-rata based on average weekly hours worked. All eligible employees can accumulate a maximum of 24 sick days; which are paid at 7/8ths of your scheduled workday.
