and • Monitor and manage the use of their allotted vacation and personal time; and • Arrive at work promptly each scheduled workday and work all scheduled hours. Requests for time off should be made as far in advance as possible to minimize disruption of the workday. If advance notification and approval is not possible, it is the obligation of each employee to personally notify his/her immediate Manager (or designated alternate) by the start of the shift if s/he is unable to report to work. The call-in report should include the following information: • Employee name. • Shift (if appropriate). • Reason for the absence. • Whether employee will use a vacation day or personal day, if eligible. • Indication of when the employee will return to work. For each day of absence, employees must personally notify their manager unless the absence has been requested and approved for a specific duration. Consideration may be given for extenuating emergency circumstances that prevent employees from giving personal or timely notice. If employees cannot return to work when expected, it is their obligation to notify their manager. When employees return to work after an absence of three (3) or more consecutive days due to their illness, injury, or hospitalization; the Company may require a doctor’s return-to-work authorization indicating they can safely return to work. When required, employees will not be allowed to return to work until the authorization report is received.

It is important to have our employees at work on a timely basis when scheduled. This policy has been adopted to assure equitable application of the rules for all employees. ITU AbsorbTech recognizes the importance of consistency in dealing with employee attendance and time clock issues. ITU AbsorbTech requires all employees to report for work on time and to work all scheduled hours and any required overtime. Excessive tardiness and poor attendance disrupts work and business operations and will not be tolerated. Poor attendance as defined by this policy is dealt with through firm but fair disciplinary action. • You are expected to report for work on time on a regular basis. Excessive tardiness and absenteeism is disruptive and places an unfair burden on other employees and your Manager. • If you must miss work due to illness or unexpected problems, you must notify your Manager as far ahead of time as possible. You should make this call personally - do not ask or rely on a friend or another employee to notify your Manager. • Excessive absenteeism or tardiness is cause for disciplinary action, up-to and including suspension and/or termination. Each employee is personally responsible to be on the job and on-time for all scheduled workdays.
