iNFINITY DENTAL IMPLANT Lifetime Warranty

The ACE Surgical Implant Warranty is designed to support our customers who place infinity Dental Implants and prosthetic products. This warranty addresses all aspects of the surgical procedure as long as either an infinity implant or restorative component was used in conjunction with the failed dental implant. The infinity Dental Implant Systems Warranty covers: Same Day Spinners ACE Surgical will replace any infinity Dental Implant that for any reason could not be placed at the time of surgery. Post-Placement, Pre-Loading Implant Failures ACE Surgical will provide you with: • An identical infinity Dental Implant as a replacement. • A $150 product credit to be used to purchase any ACE regenerative product, sutures or instrumentation required to graft the implant site.* • A replacement for any implant that fails post-loading. • A $150 product credit to be used to purchase any ACE regenerative product, sutures or instrumentation required to graft the implant site.* • One additional infinity Dental Implant and corresponding restorative components. Post-loading failure using any infinity restorative component on another manufacturer’s matching configured implant ACE Surgical will provide you with: • An identical infinity restorative component. • A matching configured infinity Dental Implant. Post-Loading Failures ACE Surgical will provide you with:

STERILE PRODUCT RETURN POLICY Unopened sterile products must be returned to ACE Surgical within 30 days of the purchase date. A 20% restocking fee will be applied to unopened merchandise returned after the 30 day purchase period.

MAKING A WARRANTY CLAIM

1. Call our Customer Experience department to report a complaint and obtain an Implant Warranty Form. 2. Complete the Implant Warranty Form and follow all instructions included. 3. You will be sent a replacement implant or prosthetic component for which you will be temporarily invoiced. 4. Decontaminate all products before returning them to ACE Surgical.

5. Submit the following three items to ACE Surgical:

1) a radiograph of the compromised site

2) the completed Implant Warranty Form

3) the decontaminated products

6. You will receive a credit for the implant and/or prosthetic component replacements upon ACE Surgical’s receipt of the implant, radiograph and Implant Warranty Form and processing of the claim.
