Preschools must provide all parents or guardians copies of the following documents:  Admission agreement  Parent Handbook

 Notification of Parents’ Rights form (LIC 995), which must also be displayed in a prominent, publicly accessible area of the preschool at all times; 41  Caregiver Background Check Process form (LIC 995E); 42  Personal Rights form (LIC 613A); 43  Identification and Emergency Information (LIC 700); 44  Consent for Emergency Medical Treatment form (LIC 627); 45  Child’s Preadmission Health History – Parent’s Report (LIC 702); 46  Physician’s Report – Child Care Centers (LIC 701); 47  Immunization requirements; 48  Copies of any licensing report that documents a Type A citation (includes facility visits and substantiated complaint investigations);  Copies of any licensing documents pertaining to a noncompliance conference between DSS and the preschool;  Copies of a summary of any accusation indicating DSS’ intent to revoke the facility’s license; and  Copies of any of the above listed licensing documents the preschool has received in the prior 12 months should be provided to parents/guardians of newly enrolling children.
