Pathlights offers HomeMeds to help older adults who: • Receive prescriptions from multiple doctors. • Take over-the-counter medications. • Experience frequent falls or dizziness. • Had a recent hospitalization/ER visit. How does it work? • An inventory of prescriptions, over-the-counter medications, and any supplements are entered into a web-based tool. • The program assesses the inventory for duplications, NSAIDS (such as Ibuprofen and Tylenol), and other potential medication risks. • A pharmacist reviews the HomeMeds analysis and identified risks. • Your prescribing physician is notified by the pharmacist of risks and the need to act on behalf of their patient.
