well-being committee of the City of Spencer employees will focus on physical, social, and emotional health. The motto for the well-being advisory committee is “Pathway to Wellness”.

HEALTHY LIFE PROGRAM The City of Spencer Employees Healthy Program applies to all Employees and Spouses on the City of Spencer Health Insurance Plan. Requirements are that each Employee and Spouse on the City’s Health Insurance Plan are required to:

1. Complete Metabolic Syndrome Biometric Screening each year. 2. Annual Physical

RETIREMENT HEALTH SAVINGS PROGRAM The City of Spencer has established a Retirement Health Savings Program through ICMA RC. Participation is set up by groups of employees. Members have mandatory deposit of accrued sick leave and vacation leave owed at retirement deposited into their RHS account. Groups may be added with approval of the Spencer City Council and must meet the requirements of the Retirement Health Savings Plan through ICMA RC. CONTINUATION OF MEDICAL AND HOSPITALIZATION INSURANCE Employees who retire before attaining age 65 years, or eligible for Medicare and their dependents, may be allowed to continue on the City's group medical/hospitalization plan, at the employee’s expense. The City has adopted the Iowa Governmental Health Care Trust 509A Governmental Retirees Plan. See plan for details. The employee and their dependents may also elect to continue medical/hospitalization benefits under the City group plan, under COBRA provisions. This election is made at the time the employee or their dependent loose group medical/hospitalization coverage under the plan, because of termination of employment, reduced work hours, employee’s death, employee's divorce, etc. (See specific COBRA provisions).

While on unpaid leave or on layoff, the employee may continue their Health Insurance at their own expense, if allowed by the insurance carrier. Premiums must be paid directly to the City
