Improving advance care planning for people with Chronic Obstructive Pulmonary Disease: The CONCORDAT (inCOrporatiNg ACP intO Routine COPD mAnagemenT) research group

The Story: Chronic Obstructive Pulmonary Disease (COPD) entails gradual deterioration in health over a long course of illness allowing and necessitating adequate planning of future care. Studies have shown that Advanced Care Planning (ACP) has a significant impact on place of death and patient satisfaction with care. To facilitate ACP in patients with COPD, healthcare professionals (HCPs) need knowledge, confidence and competence. The CONCORDAT project is a collaboration between University College Cork (UCC), Respiratory Medicine at Mercy University Hospital (MUH), Marymount University Hospital and Hospice, COPD Support Ireland, Irish Hospice Foundation (IHF), All Ireland Institute of Hospice and Palliative Care (AIIHPC) and Hospice Friendly Hospitals Programme St James’ Hospital Dublin. The group was set up to address the knowledge and skill gap of HCPs regarding the palliative care needs of people with COPD with the support of a medical education grant from GSK Ireland. The CONCORDAT group aims to design, deliver and evaluate an evidence based educational guidance document and online module for HCPs. To ensure these tools address the educational needs of HCPs, an educational needs assessment (ENA) was conducted together with a cross-sectional national survey of GPs, respiratory and palliative care nurses/ physicians and physiotherapist. Two workshops were held in October 2018, in Cork and Dublin facilitated by Dr Karen Detering, a respiratory physician, ethicist and international authority on advance care planning in COPD, the workshops were attended by over 40 HCPs.

At present there are three peer review publications being processed and three posters presentations: UCC New Horizons Research Conference 2018, the Irish Association for Palliative Care Conference 2018 and the 7th International Advance Care Conference in Rotterdam on the 13th March 2019. The learning module and associated guidance document will be available to HCPs from Spring 2019 and will be hosted, primarily on the AIIHPC learning hub.
