ANNUAL PARENT NOTIFICATION OF THE SCREENING PROCESS FOR THE CARROLL COUNTY PUBLIC SCHOOLS’ HEALTH AND CHILD FIND PROGRAM The existing Virginia Board of Education Regulations and Carroll County School Board Policy require the following: 1. Hearing and vision screening will be conducted within 60 administrative days of the opening of the school year for all students in grades K, 3, 7, and 10. Hearing and vision screening will be conducted within 90 administrative days of opening of the school year for students in Pre K. 2. All students (through grade three) within 60 administrative days of initial enrollment in Virginia Public Schools shall be screened in speech, voice, and language. 3. All students (through grade three), within 60 administrative days of initial enrollment in Virginia Public Schools shall be screened for fine and gross motor functions. 4. If a teacher has concerns about a student’s vision or hearing, this will be screened. Carroll County Public Schools Student Handbook at the beginning of each school year or upon enrollment serves as parent notification. 5. A student’s pre-school physical examination required under the Code of Virginia may be accepted for the screening if the area(s) of screening to be conducted were included and documented on the physical examination form. 6. If a student fails a screening, you will be notified by the person conducting the screening. 7. Carroll County Public Schools will designate persons responsible for ensuring that students are referred to the special education administrator or designee if results from the screening suggest that a referral for evaluation for special education and related services is indicated. The designated persons will ensure that the referral includes the screening results.

ANNUAL NOTIFICATION TO PARENTS OF STUDENTS IN GRADES 5-10 OF SCOLIOSIS INFORMATION

Scoliosis is a lateral curvature of the spine which has an increasing prevalence during the 10 th – 14 th years of age. Screening for scoliosis is recommended during this time, especially for those with a family history of the condition. Mild curvatures often need only monitoring, but some curves may need further treatment such as bracing or surgery. It is recommended that students have this screening at the health department or the family physician’s office. The screening process involves looking at the student’s back with the shirt removed, in both an erect and a forward bend position.

ANNUAL NOTIFICATION TO PARENTS OF STUDENTS IN GRADE 6 ON HUMAN PAPILLOMAVIRUS The 2007 General Assembly passed legislation that requires the Virginia Department of Health to provide parents/guardians of rising sixth-grade girls with information on the Human Papillomavirus (HPV) and the Human Papillovavirus Vaccine. The Centers for Disease Control and Prevention (CDC) recommends that girls 11-12 years old receive the vaccine. In 2011, the CDC provided additional recommendation that boys 11-12 years old also receive the HPV vaccine. The Virginia Department of Health makes electronic copies of the HPV letter to parents/guardians of rising sixth-grade students available in English and Spanish from the following website: http://www.vdh.virginia.gov/imunization/requirements/
