Techniques for reduction of cardiac toxicity IMRT or inspiration gating?

Patients with unfavorable thoracic anatomy:  Improved sparing of the heart with IMRT at cost of increased dose to the normal tissue (e.g. contralateral breast)  Sparing of the heart can be more efficient with 3D_DIBH than with IMRT_FB.
