SKILL 7-2 Assisting the patient with oral care

 1. Identify the patient.  2. Check the medical order or clinical pathway/care plan. Review the chart for any limitations in physical activity or contraindications to the procedure.  3. Introduce yourself, explain the rationale for the procedure to the patient and family, and obtain consent.  4. Gather the necessary equipment. Equipment: toothbrush, toothpaste, kidney dish or emesis basin, glass with cool water, towel, mouthwash, dental floss, washcloth or paper towel, lip balm.  5. Undertake a risk assessment, if indicated.  6. Perform hand hygiene.  7. Put on personal protective equipment (PPE), if indicated.  8. Ensure that patient privacy is maintained.  9. Place the bed at an appropriate and comfortable working height. 10. Lower the bed side rail and assist the patient to a sitting position, if permitted, or turn the patient onto their side. Place a towel across the patient’s chest. 11. Encourage the patient to brush their own teeth, or assist if necessary. (a) Moisten the toothbrush and apply toothpaste to the bristles. (b) Place the brush at a 45-degree angle to the gum line and brush from the gum line to the crown of each tooth. Brush the outer and inner surfaces. Brush back and forth across the biting surface of each tooth. (c) Brush the tongue gently with the toothbrush. (d) Have the patient rinse vigorously with water and spit into the emesis basin. Repeat until clear. Suction may be used as an alternative for removal of fluid and secretions from the patient’s mouth.
