16. Remove and dispose of dirty gloves and put on clean ones. Use clean linen and make the near side of the bed. Place the bottom sheet with its centre fold in the centre of the bed. Open the sheet and fan-fold to the centre, positioning it under the old linen. Pull the bottom sheet over the corners at the head and foot of the mattress. 17. If using, place the drawsheet or protective underlay with its centre fold in the centre of the bed and positioned so it will be located under the patient’s midsection. Open the drawsheet and fan-fold to the centre of the mattress. Tuck the drawsheet securely under the mattress. If a protective pad is used, place it over the drawsheet in the proper area and open to the centre fold. Not all facilities use drawsheets routinely. The nurse may decide to use one. 18. Raise the bed side rail. Assist the patient to roll over the folded linen in the middle of the bed towards you. Reposition the pillow and blanket or top sheet. Move to the other side of the bed and lower the bed side rail. 19. Loosen and remove all bottom linen. Discard soiled linen in the laundry bag or hamper. Do not place on floor or furniture. Do not hold soiled linen against your uniform. 20. Remove gloves if worn and undertake hand hygiene. Ease clean linen from under the patient. Pull the bottom sheet taut and secure at the corners at the head and foot of the mattress. Mitre the corners if using a flat sheet. Pull the side of the sheet taut and tuck under the side of the mattress. Repeat this with the drawsheet. 21. Assist the patient to turn back to the centre of the bed. Remove the pillow and change the pillowcase. Open each pillowcase in the same manner as you opened other linen. Gather the pillowcase over one hand towards the closed end. Grasp the pillow with your hand inside the pillowcase, keep a firm hold on the top of the pillow and pull the cover onto the pillow. Place the pillow under the patient’s head, with the open end facing towards away from the door. 22. Apply top linen, sheet and blanket, if desired, so that it is centred and hems are even with the head of the mattress. Fold the top linens over at the patient’s shoulders to make a cuff. Have patient hold on to top linen and remove the blanket, if used, from underneath.
