The [District] is conducting an investigation into certain allegations involving the workplace. The [District] has arranged for an [outside] investigator to conduct the investigation. The name of the [outside] investigator is [Name of Investigator]. You have been identified as someone who may have information relevant to the allegations. We have arranged for you to meet with the investigator and your interview is scheduled on [insert date] at [insert time] at [insert location]. You are to appear at that time to answer questions relating to this investigation. It is important that you cooperate fully during this investigation and that you answer all questions honestly and completely. [OPTIONAL: If the [District] wishes to consider giving an order that the witness keep the content of the interview confidential the [District] should obtain legal advice to evaluate whether there is an adequate factual basis to do so under the limited exceptions to the rule that blanket gag orders are unlawful. [Name of District] reaffirms its policy and commitment to providing a discrimination and harassment-free workplace. [Name of District]’s policy also prohibits persons from retaliating against another for having reported harassment or for participating in the complaint and investigation process. Please immediately report actions, comments, or conduct, if any, that you believe to be discrimination, harassment, or retaliation for your participation in this complaint and investigation process.
