K eyLogic Systems, Inc. takes pride in offering a comprehensive and competitive benefits package to full-time employees. KeyLogic, through all of its benefit partners, offers you a benefit program that allows choice and flexibility. Employees are eligible for all health benefits, beginning on their first day of employment. It is important that you take the time to review all of

the plan options available to you. Consider each benefit and the associated cost carefully and choose the benefits package that will best meet you and your family’s needs throughout the year. Options selected upon hire remain in place through the end of the plan year. The plan year for KeyLogic is February 1st through January 31st of the following calendar year. The Internal Revenue Service (IRS) states that eligible employees may only make elections to the plan once a year at open enrollment. Historically, KeyLogic has held Open Enrollment in the month of January, for a February 1st effective date. This means that Medical, Dental, and Vision benefit choices are binding through January 31 st of each year. The following circumstances are the ONLY reasons you may change your benefits during the year:

Birth & Adoption Loss of Dependent Status Loss of Spouse’s job where coverage is maintained through a spouse’s plan

These special circumstances, often referred to as qualified events, or life status changes, will allow you to make plan changes at any time during the year in which they occur. For any allowable changes, you must inform the Human Resources Department within 30 days of the event to avoid lapse in coverage. All other changes are deferred to open enrollment.

The KeyLogic medical options are designed to provide you and your family with access to high quality and affordable healthcare. Two plans are available through Cigna to provide the employees of KeyLogic even more flexibility and savings in their medical coverage options. There is an OAP2 plan available (FSA eligible) as well as a high- deductible (HDHP) plan (HSA eligible—see page 3). The medical options cover a broad range of healthcare services and supplies, including prescriptions, office visits and hospitalizations. The plans differ when it

comes to how costs are shared with you. Please refer to the summary on page 2 for specific details concerning plan design. A schedule of monthly medical premiums is also included on page 7.
