3. (a) Write one (1) goal for your client, including the agreed actions and responsibilities for the: (i) client (ii) case manager. (b) Explain how the goal is Specific i.e. Who is involved? What will the goal accomplish? Where will it be done? How will it be achieved? Why – what are the reasons and purpose? Which constraints and requirements apply? (c) Explain how the goal is Measurable i.e. How will you know if the goal has been reached? Can you track the progress and measure the outcomes? (d) Explain how the goal is Achievable and Attainable i.e. Is it possible? Does the client have the necessary knowledge, skills, abilities and resources to accomplish the goal? (e) Explain how the goal is Relevant and Realistic i.e. What is the reason, purpose or benefit of accomplishing the goal? Can it be done? Is it worthwhile? Will it meet the client’s identified need? (f) Explain how the goal is Timely and Time-bound i.e. Is there a timeline in which the goal is to be realised? Does it create a practical sense of urgency? (g) Re-write your initial goal using the S.M.A.R.T criteria.
