(b) List the actions required to monitor compliance by the: (i) client (ii) case manager (iii) key stakeholders (iv) agency/program. (c) List the agency/program and funding body guidelines that you would follow if you identified any non- compliance.

3. (a) List the steps you would undertake to identify the need for an adjustment/variation to the client’s care plan e.g. resources, services and supports. (b) Describe how you would justify the need for the identified change. (c) List the tools and documents that you may use during this process. (d) Explain how you would involve the client and/or key stakeholders in this process. (e) Explain the purpose and rationale for involving the client and/or key stakeholders in the process.
