Eligibility: Patients must be a resident of Fond du Lac or Dodge County, must have no insurance, including Medicaid, Medicare or BadgerCare. Must have an income at or below 165 percent of the federal poverty level and must be 19 years of age up to 64 years of age.

The Save a Smile Children's Dental Program meets the basic preventive and restorative dental needs of Fond du Lac County residents, 18 years and younger, who only have Medical Assistance insurance. The Adult Save a Smile Program is able to assist Fond du Lac County residents needing urgent dental care. To be eligible one must be 19 years and older, who only have Medical Assistance coverage or meets required income level.

Programs and services: Vouchers for nutritious food for income eligible pregnant, postpartum women and children under 5 years of age. Also provide nutrition sessions, counseling, health screening and referral to other health services.
