Step Six Fill out the online Chapter Completion Form for your approved SIAFU Chapter (and pay the one time, Chapter start-up fee).* *The one-time, Chapter start-up fee is calculated on a sliding fee scale based on the approximate number of individuals your church or organization serves on a monthly basis. This is an honor system; whatever estimate you provide will be accepted, and you can then pay the fee associated with your guestimate. The scale is below:

• 1-50 individuals served monthly . . . . $50

• 51-100 individuals served monthly . . . . $75

• 101-250 individuals served monthly . . $100

• 251 and up individuals served monthly . . $150

Please note, there is an annual Chapter renewal fee of $25 along with your continued affirmation of our Statement of Faith (due January 31 after the first full year of membership). The Checklist will help you think through the various elements you’ll want to plan in starting your Chapter. Once you have thought through these important matters for your Chapter, you’ll be ready to submit your Chapter Completion Form and pay the setup fee. Step Seven Finally, receive your user name and password to the SIAFU Network Chapter website. Once you have submitted the Chapter Completion Form and paid your fee, we will send you a user name and password for the SIAFU website (immediately emailed once we receive your form), and will sign you up for the SIAFU monthly newsletter.
