C HECKLIST F OR L EAVE O F A BSENCE F OR E MPLOYEE ’ S B ABY B ONDING /A DOPTION /F OSTER C ARE

The following is a checklist to employ when ascertaining whether to approve a leave of absence for a qualifying event, reinstating the employee upon expiration of the leave and for removing the employee from active service if the employee is unable or unwilling to return from the leave of absence. 1. Employee Name: _______________________________________________ 2. Determine whether employee has exhausted pregnancy disability leave? Employee’s PDL leave has ended Employee is still on PDL leave Not applicable NOTE: If the employee is still on PDL, do not run the employee’s CFRA leave. 3. Is the employee taking time off for prenatal appointments? Yes No 4. Has or will the employee be absent to bond with newborn or child placed with employee in connection with adoption or foster care? Yes No 5. Evaluate whether the employee is eligible for FMLA and/or CFRA bonding leave. Employee is eligible for FMLA/CFRA bonding leave Employee is not eligible for FMLA/CFRA bonding leave If the leave of absence and/or the employee do NOT qualify for FMLA and/or CFRA bonding leave, skip to No. 13 below.
