 Whether the requested accommodations will allow the student to effectively access and participate in the course or program.  Whether the requested accommodations will alter the essential requirements of the course or program.

 Acceptable sources of information and documentation for substantiating a student’s disability and request for particular accommodations include:  Student’s/parent’s self-report.  Observations and interactions during interviews and conversations with the student.  Information from third parties – e.g., educational or medical records, reports and assessments created by health care providers, school psychologists, or teachers.  Accommodate by making reasonable modifications in policies, practices, or procedures.  Need not eliminate academic requirements essential to the academic program.  Reasonable modification may include:  Modification to examination:  Providing more time.  Conducting exam in distraction-free testing room.  Providing a reader.  Providing a tutor is not necessary unless tutoring services are provided to non-disabled students.  Providing auxiliary aids and services: o Note takers. o Computer-aided transcription services. o Interpreters. o Assistive listening devices. o Closed caption decoders. o Telecommunications devices for the deaf (TDDs). o Readers. o Taped texts. o Audio recordings. o Braille materials. o Large print materials.
