Allegations have been made that you engaged in conduct that violates School rules including [Identify applicable rules] and the School’s expectation for [insert, e.g. faculty members]. The [Name of School] is conducting an investigation into these allegations. The purpose of the investigation is to gather the relevant facts. The School has arranged for [Name of Investigator], an outside investigator, to conduct this investigation. You will be given an opportunity to speak with the Investigator. Your interview is scheduled on [insert date] at [insert time] at [insert location]. You are expected to answer the Investigator’s questions honestly and thoroughly. Please appear at that time to meet with the Investigator. You will be excused from your duties to attend the interview. [OPTIONAL: If the school wishes to consider giving an order that the witness keep the content of the interview confidential the school should obtain legal advice to evaluate whether there is an adequate factual basis to do so under the limited exceptions to the rule that blanket gag orders are unlawful.] [Name of School] employees are prohibited from engaging in retaliation.
