investigation for employment purposes and that the school may obtain an investigative consumer report.  Provide the notice concurrently with the job application. The notice should disclose that the investigative consumer report may contain information about the applicant’s character, general reputation, personal characteristics, mode of living, and medical information.  Provide an election form concurrently with the job application. The applicant should indicate whether he or she wants to receive a copy of any investigative consumer report.  Is the election form completed and signed by applicant? Steps if conducting a reference check in-house  At a minimum, the reference check should verify the following information about applicants:  Dates of employment;  Job duties and positions held;  Quality of work;  Quantity of work;  Ability to work with others;  Ability to take direction;  Leadership skills;  Attendance;  Behavior;  Any performance problems;  Whether the applicant was ever asked to resign; and,  Reason(s) former employment ended.  If the background investigation will be conducted in-house, ensure that each applicant has been provided with notice that he or she has the right to obtain a copy of any public records obtained.  Provide the notice and an election form concurrently with the job application. The applicant should indicate whether he or she wants a copy of any public records obtained.  Is the election form completed and signed by applicant?  Contact all persons to verify key information about the applicant’s employment history and work performance.  Direct Supervisors.
