C HAPTER 5 – E MPLOYEE E VALUATIONS D OCUMENT 5.A C HECKLIST F OR E VALUATIONS  Establish your process, including type of evaluation and dates  Draft and develop evaluation form  Make sure process is consistent with any policy/process in employee handbook and employee contract. If the employee handbook or employee contracts mention the employee evaluation process, make sure the policy or other language is broad enough and not overly detailed  Establish goals and expectations for employees to meet through job descriptions, individual, staff, faculty and other meetings, memos and other interactions  Create separate interim file to use throughout the year for contemporaneous notes and documentation  Set up classroom observation dates  Continue ongoing communication throughout the year regarding performance, including memos, meetings, and notes to file  Schedule annual evaluation meeting. If self-evaluation is required, make sure employee has time in advance to complete the form  Draft formal evaluation  use specific and descriptive language  be direct and honest  concentrate on behavior and actions, not personality  focus on impact of behavior, actions or inaction on students, parents or others being served by the position  refer to established goals  identify any and all performance deficiencies  praise positive performance and growth  Conduct evaluation meeting with employee  Follow up meeting with any needed training, mentoring or other assistance or professional development.
