S AMPLE N OTICE P LACING A CCUSED E MPLOYEE O N P AID A DMINISTRATIVE L EAVE P ENDING A DMINISTRATIVE I NVESTIGATION

[Date] SUBJECT: Administrative Leave Effective immediately, you are placed on administrative leave with pay pending completion of an investigation regarding [brief description of allegations]. Until further written notice from me, you are relieved of duty and are not authorized to represent yourself as an employee of [Name of school] or to perform any duties of your job while on paid administrative leave. You are not authorized to enter upon [Name of school] grounds, unless I so directed or otherwise consent to your presence. While on administrative leave your regular working hours shall be considered Monday to Friday from [specify work hours]. Each day at 9:00 a.m., you shall contact me at [phone number] to receive any further direction. You must also provide me with a telephone number where you can be reached during your regular working hours while you are on administrative leave. You shall promptly return any messages that I may leave for you. If there is a reason why you cannot be available by telephone during your regular working hours, you must contact me to make appropriate arrangements. As you have been advised, an investigation is being conducted into allegations that you violated the School’s policy prohibiting harassment, discrimination and retaliation, specifically that you engaged in conduct that violates the prohibition on [insert relevant information]. You shall make yourself available during your regular working hours to participate in interviews or otherwise cooperate with the investigator. You are ordered to cooperate fully in the investigation and to be completely truthful. As you have also been advised, School policy prohibits persons from retaliating against another for having reported possible violations of School policies and standards of conduct or for participating in the complaint and investigation process. To that end, you are ordered not to take any actions that might be viewed as retaliation against anyone whom you suspect of participating in the investigation process, either as a complainant or witness. Disobedience of these orders will be considered insubordination and may lead to disciplinary action up to and including termination.
