The 2019 Program Year Has Begun!

IMPORTANT DATES

DECEMBER 31, 2018 Deadline to complete gateway activities. Your Wellness Questionnaire must be done and the results of your Biometric Screening received by Quest Diagnostics Lab in order to qualify for the 2020 incentive. AUGUST 31, 2019 Deadline to earn an additional 30 Wellness Credits (total 50).

SPOUSE/DOMESTIC PARTNER INCENTIVE INCREASE | The spouse/domestic partner incentive will increase to $50 per month in 2020! Participate this year and see double the savings in 2020! WEIGHT WATCHERS | Get the Reliance discount of 50% off select Weight Watchers offerings and earn up to 10 Wellness Credits for participating. DAILY DASH | Complete daily challenges in four topic areas: Eat Well, Move More, Stress Less, and Sleep Tight. Record 10 Daily Dashes to complete one cycle and earn 5 Wellness Credits (15 maximum). TOBACCO-FREE PLEDGE | Commit to be tobacco free! Complete all three tobacco cessation program components and earn 20 Wellness Credits. EAP WELLNESS WEBINARS | We’ve integrated our Anthem Employee Assistance Program (EAP) webinars with wellness! Earn 5 Wellness Credits (15 maximum) when you watch an online EAP Wellness Webinar, complete the survey, and self-report your participation. New topics offered each month. UNIFY | Get educated about health and well-being topics via online webinars. Two topics each month; the format allows for ease of on-site group implementation. Earn 5 Wellness Credits (15 maximum) for each webinar you view.
