SCHOOL HEALTH REQUIREMENTS AND SERVICES (JHCB)

Virginia law requires that no pupil shall be admitted for the first time to any kindergarten or elementary school in a school division unless he/she: (a) has a report from a Licensed Physician, Licensed Nurse Practitioner, or Licensed Physician Assistant of a complete physical exam performed within 365 days of entrance to a Virginia Public School or (b) provides records showing that the pupil furnished such a report prior to entering another school division and include the information contained in that report. Virginia law also requires that, at the time of admission of a student, documentary proof of immunizations meeting Virginia School Immunization Requirements be provided. The documentary proof must include the month/day/year of administration of required vaccines and must be signed by a physician or his designee or an official of a local health department. This proof must be on an official health department or physician office form. A registered nurse provides school health services in each school on a daily basis. Some of these services include screenings, medication administration, health and family life education, sudden illness/emergency care, care of medically fragile students, and school/community health care liaison. In case of medical emergencies, the principal, school nurse, or other designated personnel may call 911 if deemed necessary. Every attempt will be made to contact the parent/guardian or other designated emergency contact as soon as possible. Carroll County Public Schools partner with the Smile Programs to provide dental services to students who qualify. School nurses coordinate and schedule the Smile Programs at each individual school.

SCHOOL HEALTH RECORDS (JOA-R)

Medical records and sensitive medical information not relevant to the student’s educational process shall be kept in the school nurse’s locked files and shall be accessible only to the school nurse or nurse’s substitute. Relevant medical records shall be sent in a separate confidential file to the school nurse when a student transfers to another school in Carroll County. Written, specific parental consent is required for release of school health records not part of the Student Education Record to be sent outside the school district. The information in the above named records is not to be shared with other school personnel except with written parental consent or when necessary to meet the student’s health and educational needs. Personal nursing notes kept by the nurse shall be retained by the nurse as allowed by FERPA. Except for the records referenced in the Student Education Record notice or specifically referenced in the Library of Virginia Records Retention and Disposition Schedule, GS 21, all health records shall be retained for five years at the school last attended and then destroyed.
