As we grow older, many of us want to continue living in our own homes but may need some assis tance to remain independent. The IRIS program (Include, Respect, I Self-direct) is Wisconsin’s fully self directed option for seniors and individuals with disabilities who qualify for Medicaid. With IRIS, you decide what care and services you receive. This means you live life on your own terms while also having trusted support by your side. With IRIS, you have many choices and get to choose how you want things done. When you join IRIS, you are given a budget based on your unique needs and goals. The best part is that you decide how to use your budget, by choosing: How it Works • The services and supports you need to stay indepen dent • When and where you want your supports provided • The people you trust to pro vide care, including family or friends • How you manage your IRIS budget effectively and wisely Everyone in IRIS has a support and service plan. This plan is made just for you, based on your long-term goals and what you need to live well. Your plan can include things like getting meals delivered to your home, making changes to your house so that it’s more accessible (like adding ramps), and finding help with transportation to make everyday life easier.
