Our team wants you to know: ’ Hospice care is provided wherever you call home – in your own residence, at a care facility or our Inpatient Center. ’ A hospice referral can come from you, a family member or physician - anyone who cares about the well-being of the patient. Our consultations are free, and we will meet wherever it is most convenient for you. ’ Hospice supports and cares for the entire family. Our hospice team is focused on the physical, emotional and spiritual needs of the patient, family and other caregivers. ’ You have the right to choose your hospice provider.
