Chemical Dependency Treatment

As used in Questions 1 through 6, “clear and convincing evidence” means that measure or degree of proof that produces a firm belief or conviction that the allegations sought to be established are true. QUESTION 1 Do you find by clear and convincing evidence that INITIALS OF PRO POSED PATIENT is a person with a chemical dependency? “Chemical dependency” means the abuse of alcohol or a controlled sub stance, psychological or physical dependence on alcohol or a controlled sub stance, or addiction to alcohol or a controlled substance.
